Dear Valued Patient,

I am writing to inform you that I will be leaving North Texas Musculoskeletal Medicine (NTXMSK) to open my own clinic. My last day with NTXMSK will be March 31, 2025. Please rest assured that I will continue to provide the same high-quality care throughout this transition period.

In accordance with the Texas Medical Board (TMB) Rules and the Texas Administrative Code, I want to ensure you are informed about your options for continuing care and accessing your medical records:

1- Continuity of Care: I will remain available to provide necessary care until my departure. After March 31, 2025, I can be reached for non-emergency concerns at:

2- Medical Records Access: Your medical records will remain confidential and securely maintained by NTXMSK. If you wish to transfer your care to my new clinic or another physician, you may request the transfer of your records in writing by contacting NTXMSK at:
